This Non-Disclosure Agreement (the “Agreement”) is entered into at the time of submission of this form, by and between JSE “Company” and the submitter “Applicant”. The Applicant is applying for a position with the Company and will receive certain confidential information during the recruitment, application, interview, and contractual process. The Applicant agrees to protect this information as set forth below. “Confidential Information” means any and all information disclosed by the Company to the Applicant, whether orally, in writing, or by any other means, including but not limited to: • Details of the recruitment and hiring process; • Activities, discussions, questions, or materials presented during applications, interviews, or contract negotiations; • Names, locations, phone numbers, email addresses, titles, and any other personal or contact information of the Company’s staff, employees, contractors, clients, or other contacts; • Any other business, operational, or proprietary information provided during the recruitment process. The Applicant agrees to: • Hold all Confidential Information in strict confidence and not disclose it to any third party without the prior written consent of the Company; • Use the Confidential Information solely for the purpose of evaluating and pursuing employment with the Company; • Take reasonable precautions to prevent unauthorized disclosure or use of the Confidential Information. This Agreement does not apply to information that: • Is or becomes publicly available through no fault of the Applicant; or • Is already known to the Applicant prior to disclosure by the Company. • This Agreement remains in effect for a period of two (2) years from the date of last disclosure of Confidential Information, or until the Company provides written notice of termination, whichever occurs first. The submission of this application with the applicants typed signature (digital signature) constitutes agreement and adherence to the terms of this Non-Disclosure Agreement.

This Statement is entered into voluntarily at the submission of this application by and between JSE “Company” and the submitter “Applicant” or “Participant”. The Applicant wishes to participate in consensual intimate and/or sexual activities as part of entertainment content, productions, performances, or media created by JSE. This Statement documents the voluntary and enthusiastic consent of the Applicant to engage in such activities. The Applicant hereby declares and affirms the following: • I am participating in all intimate, sexual, or adult-oriented activities freely, voluntarily, and with full consent. • I confirm that my consent is given without any coercion, pressure, duress, intimidation, or expectation of employment benefits or penalties. • I understand that these activities are for entertainment, media, content creation, and/or artistic purposes owned and controlled by the Company. • I have the absolute right to withdraw my consent at any time, for any reason, even during an activity or scene, and such withdrawal will be respected immediately without retaliation. • I agree to clearly and promptly communicate any boundaries, limits, safe words, or discomfort during any activity. This consent applies to intimate activities including, but not limited to: kissing, touching, oral sex, vaginal sex, anal sex, BDSM elements, group activities, or any other sexual acts specified in advance for the Company’s entertainment productions. Specific scenes or acts will be discussed and agreed upon prior to filming or performance. Participation in these activities may be part of the recruitment process. Consent or refusal to participate may affect the Applicant’s consideration for any position with the Company. Professional conduct will be maintained outside of designated entertainment activities. The Applicant understands that the resulting content may be published, distributed, or sold by the Company. The Applicant grants the Company the right to use their likeness and performance in such content, subject to any separate talent/release agreement. The Applicant confirms they are participating responsibly and will comply with all health, safety, and testing protocols required by the Company.

As part of our legal compliance and age verification process, the Company may collect the following personal data from applicants, participants, or talent: • Full legal name • Date of birth • Government-issued identification (e.g., Driver’s License, State ID, Passport, or other valid photo ID) • Any other information required to verify identity and legal age This information is collected solely for the purpose of: • Verifying the individual’s identity, and • Confirming they meet the legal age requirement (18+ or 21+ as applicable) for participation in adult-oriented or regulated entertainment content. The data will not be used for any other purpose, including marketing, employment decisions, or sharing with third parties except as required by law. JSE takes the protection of your personal data seriously. All collected identification documents and personal information will be: • Stored securely using industry-standard encryption and access controls; • Accessible only to authorized personnel on a strict “need-to-know” basis; • Retained only for the minimum time required for legal compliance and verification purposes. We do not sell, rent, or otherwise distribute your personal identification information to unrelated third parties. In the event of any data breach, we will notify affected individuals and relevant authorities as required by applicable law. Upon request (where legally permitted), individuals may inquire about the storage and deletion timeline of their verification records. By providing your identification documents to JSE, you acknowledge and consent to the collection and secure storage of this information strictly for the verification purposes outlined above.

This Electronic Signature Agreement is entered into at the time of acknowledgment and submission by and between Jason Strong Entertainment (“Company”) and (“Applicant”) The parties agree that: 1. Any typed name, electronic signature, or digital mark (including but not limited to a typed full name in a signature field, DocuSign, Adobe Sign, or any other electronic platform) on this Agreement or any related documents shall constitute and have the same legal force and effect as an original handwritten signature signed in person. 2. The use of a typed name or electronic signature represents the signer’s intent to be legally bound by the terms of the document as if it were physically signed. 3. Each party agrees that their typed name appearing on this document or any associated forms is their legal signature and is made voluntarily with full understanding and acceptance of all terms. 4. This acknowledgment satisfies the requirements of the U.S. Electronic Signatures in Global and National Commerce Act (ESIGN) and the Uniform Electronic Transactions Act (UETA), as well as any applicable state laws. By proceeding, both parties consent to the use of electronic signatures and agree that this method is legally binding and enforceable.
